Signs That May Lead Someone to Seek a Swallowing Evaluation

These signs can have different causes. Appropriate medical or speech-language evaluation may be needed to understand what is contributing.

    Coughing During Meals

Coughing or choking while eating or drinking may signal a swallowing concern.

    Throat Clearing

Frequent throat clearing with eating or drinking can be a sign to explore.

    Difficulty Swallowing

A feeling that food or liquids are difficult to swallow should be evaluated.

    Taking Longer to Eat

Meals taking much longer than usual may reflect a swallowing change.

    Difficulty With Certain Foods

Trouble managing specific foods or liquids can occur for different reasons.

    Changes in Mealtime Confidence

Avoiding certain foods or feeling uneasy at meals may be worth discussing.

    Repeated Swallows

Needing multiple swallows for a single bite or sip can be a concern.

This list is not a diagnosis. Swallowing symptoms can have different causes, and appropriate evaluation helps determine the right next steps.

What a Speech-Language Pathologist May Address

Services are provided within the clinician’s licensure and scope, based on clinical findings and individualized needs.

    Swallowing Function

Clinical assessment of swallowing-related concerns within the clinician's scope.

    Mealtime Strategies

Individualized strategies based on clinical findings and recommendations.

    Positioning & Environment

Consideration of positioning and the real mealtime environment when relevant.

    Education

Education for the client and authorized family or caregivers.

    Following Medical Recommendations

Reinforce appropriate recommendations from physicians and the healthcare team.

Treatment does not invent universal diet modifications or exercises. Strategies are individualized based on clinical findings.

Why Observing Swallowing During Real Mealtimes Can Be Helpful

Observing swallowing and mealtime in the home environment can provide context that a clinic visit may not.

    Actual seating
    Usual foods and beverages
    Familiar utensils
    Everyday distractions
    Mealtime routines
    Caregiver involvement

Home observation does not replace instrumental testing when instrumental testing is clinically indicated.

    Swallowing Changes With Neurological Conditions

Swallowing problems can occur with some neurological conditions, including stroke, Parkinson’s disease, brain injury, and other neurological conditions. If mobility or balance is also a concern, our stroke physical therapy, Parkinson’s physical therapy, and brain injury physical therapy pages address those needs, while swallowing therapy is provided through speech-language pathology.

When Additional Medical Evaluation May Be Needed

New or worsening swallowing problems may require medical evaluation, and instrumental assessment may sometimes be recommended by the appropriate clinician. This page does not provide emergency diagnosis or attempt to determine aspiration risk from symptoms. If you are concerned about swallowing safety, contact a physician or seek appropriate medical attention.
